Title: Player add screen will not dislpay last name or nick name

I can get my database to display all the names I need however my player add screen will only display first names.   This changed on me and I do not know how to change it back.

Select the name format you want by pressing the Name Format button on the Preferences tab of the Settings window.  Choose which columns you want to display on the various player dialogs by selecting the Player Details Fields Category on the Preferences tab and then pressing the Configure Columns on Player Dialogs button.
